Narrative

Rena departed Napier at 10:20 on 4 October 2011 bound for Tauranga. The master initially estimated the arrival time without fully accounting for unfavourable coastal currents. After the vessel made slower progress, he authorised the bridge watchkeepers to deviate from the planned course lines to shorten the passage and search for less adverse current.

During the night, the second mate reduced the intended clearance past Astrolabe Reef from two nautical miles to one nautical mile and made a series of small course changes. An overcorrection placed Rena on a direct track toward the reef. When the master returned to the bridge, the handover provided virtually no information about the ship’s position, heading, or immediate navigation threats. Position monitoring had broken down, and the bridge team did not recognise the developing grounding risk in time.

At about 02:14 on 5 October, Rena grounded on Astrolabe Reef in the Bay of Plenty. The initial impact severely damaged the hull. The hull girder subsequently failed and the vessel broke into two sections; the aft section moved off the reef and sank. About 200 tonnes of heavy fuel oil escaped into the sea, substantial container cargo was lost, and the vessel was declared a total loss. No fatalities or injuries were reported.

The Transport Accident Investigation Commission found no machinery or navigation-equipment failure that caused the grounding. It identified poor passage planning, inadequate navigation watchkeeping, and ineffective transfer of control as direct contributing factors. The investigation also raised wider issues involving company oversight of the safety management system, transparency of standards-of-training auditing, the availability of coastal shipping data, and the use of virtual aids to navigation.

Investigation

The Transport Accident Investigation Commission completed its investigation into the grounding. It found that the casualty was not attributable to a malfunction of onboard machinery or navigation equipment. The direct contributing factors were failures in passage planning, navigation watchkeeping, and transfer of control. TAIC also identified wider safety-management, training-audit, coastal-data, and virtual-navigation-aid issues.
