Narrative

On 31 January 2017, USS Antietam was preparing to anchor at Anchorage A-10 in Tokyo Bay, Japan. The anchoring plan did not adequately account for prevailing wind and current, and the bridge team did not maintain a reliable assessment of the ship's movement as it approached the assigned position. Antietam passed the intended anchorage by about 60 yards and continued toward shoal water. The commanding officer delayed decisive use of the engines while attempting to recover the evolution, and the ship grounded at 09:53 local time. The investigation found that the bridge and combat information centre teams did not provide effective forceful backup. Watchstanders observed the developing danger, but no one clearly and urgently stated that the ship was standing into unsafe water.
