Narrative
Panam Serena was alongside at Porto Torres and discharging cargo after arriving from northern Europe. The cargo included 2,091 tonnes of Benzene and 6,300 tonnes of Cut C6. The terminal connected the cargo and vapour-return pipelines and normally connected a ship-to-shore electrical continuity bonding cable, although the BMA found uncertainty about whether that cable had been connected correctly.
At approximately 11:55 on 1 January, near the end of the discharge, the vessel was shaken by the first of approximately four violent explosions. The explosions occurred amidships in the cargo-tank area and were followed by an intense fire. Two seamen on the main deck were killed, and the chief mate was injured. The rest of the crew escaped over the stern into the water and then into the stern freefall lifeboat, which had launched automatically. Shore-based emergency services brought the fire under control, but the damage to the vessel was catastrophic and the terminal jetty was also damaged.

Investigation
The Bahamas Maritime Authority investigated the casualty and issued its report in April 2006. The report states that the exact cause of the initial explosion could not be identified because of the catastrophic damage. Its most probable-cause conclusion was a static or electrical discharge igniting a volatile vapour mixture. The BMA also found that electrical continuity and bonding guidance had not been adequately followed by the terminal and that the crew had not verified the bonding cable's condition and connection.
The report does not establish whether the bonding cable was connected correctly at the time of the casualty, whether air or nitrogen was used during the discharge operation, or the precise point and mechanism of the initial discharge. It provides no economic loss estimate.
