Narrative
On 14 March 2004, Samantha J was assisting with a log-barge manoeuvre in Ladysmith Harbour, British Columbia, when it girded and capsized. Appendix B of TSB report M18P0230 identifies this event as M04W0045. Its brief entry gives no details of the tow arrangement, response, casualties, damage or subsequent recovery. Separate October 2005 and October 2014 sinkings are recorded under the name Samantha J; their reported circumstances cannot establish what happened in Ladysmith.

Investigation
The Transportation Safety Board of Canada records M04W0045 in Appendix B of report M18P0230 (printed page 34). That report investigates George H. Ledcor; its causal findings do not establish the causes of this occurrence. The earlier North Arm Venture report M09W0141 (page 10, footnote 22) names M04W0045 as Rivtow Capt. Bob, whereas Appendix B of M18P0230 names Samantha J. This vessel-identity discrepancy is unresolved. No standalone report for M04W0045 was located in the exact-identifier and Canadian government archive searches.
