Why it matters

The first grounding accident reports already show distinct variants of the same failure pattern:

  • Ovit: the bridge team followed an unsafe route on ECDIS and failed to act on clear navigational danger in the Dover Strait.
  • Roger Blough: the second mate did not effectively monitor the electronic chart track while the vessel straddled the edge of the channel.
  • Ever Forward: the pilot missed a planned turn while relying on a single navigation display and using personal electronic devices.
  • Beaumaiden: the bridge was left effectively unattended after the watchkeeper left and fell asleep in his cabin.
  • Kaami: the voyage plan and ECDIS setup were flawed, and passage monitoring did not catch the developing danger.

These are not identical events, but they belong in the same family of navigational control failures. Tracking them together should help later queries about distraction, lone watchkeeping, BNWAS use, fatigue, ECDIS practice, and pilot-bridge interaction.
